First. “What” are we looking for?

While we are talking about smiles, I am looking at your face, tan, and hair; I am looking how you move you lips, what part of your teeth and gums are revealed in normal movements. 1. The esthetic smile possesses asymmetry within the borders of symmetry. The smile must be symmetrical from a distance, but at closer look should be discovered slight differences: jagged cutting edges or small differences in the lengths of the upper lateral incisors.

1. Medical examination - detailed examination, plus pictures, plus X-ray images, plus a record in your personal documentation.
   
2. Plan - treatment/rehabilitation plan.
   It includes:
   • Professional hygiene,
   • Treatment of caries and gum diseases
     This is the basic phase of the plane. It execution can solve all your problems, if you like your natural and cleaned smile. My responsibility is to invite you every six months for check up, cleaning and motivation.
     If you want to, there is a reconstructive phase. It includes:
   • Whitening,
   • Ceramic facets and fillings,
   • Other prosthetic decisions.
   
   
3. Explanation – I will explain in details the plan, terms and prices.
   Sometimes the restoration takes a long time. If we get mutual consent, than follows:
   
4. Professional teeth cleaning:
   • Tartar cleaning – ultrasound, hand curette and polishing
   • External coloring agent – air-water polishing equipment.
     
     This is important because of several reasons:
     - This way I can identify your natural teeth color and can keep it in your file.
     - This is very important for the parodont – the complex of gums and bone, which supports teeth. Bleeding gums make impossible all manipulations, related to esthetics.
     
     In this phase I will teach you how to brush you teeth in order to keep your new smile, which I will check up every year.
     
5. Treatment of caries endodont (tooth nerve). The medical aspect is also important apart of the esthetic one. The procedures are under anesthesia and are not painful.
   
6. Teeth whitening:
   • If only you want change in the color.
   • If the shape is to be changed with facets, but your teeth are dark and could be seen trough.
   
   There are 20-year studies which have proved that professional whitening is gentle.
   Peroxide gels are applied in varying concentrations. The released oxygen breaks down the molecules of the coloring agents and they loose the color.
   The effect lasts 3 to 5 years, but could be expanded by additional measures.
   
   Contraindications:
   • Poor mouth hygiene
   • Fillings with defects
   • Parodontitis (parodontosis)
   • Sensitive teeth necks
   • Pregnancy

We do the following:

1. Two imprints of the jaws for plaster models.
2. Soft plastic splints, as models for gel.

When whitening is performed in home, You have to:

1. Brush teeth and splints.
2. Apply prescribed by us amount of gel
3. Place the splints and carefully clean out the excess of the gel
4. Rinse you mouth with water
5. Go in bed or continue your activities
6. Every two days you have to come for check up in order to trace you improvement and to receive next dose of gel.

IMPORTANT!!!

1. You are not to drink coffee, tea, coke, red wine, and juices.
2. Don’t smoke at all!

For 15 days.

If you can not endure it, we can perform express whitening.

1. In my cabinet – for one hour with splints on and applied gel.
2. On the chair, instantly: the gel is applied over the teeth and a special catalyser in it is activated by a photo-polymeric lamp. This procedure is prompt, but the concentration is higher and there is a risk of adverse reactions – teeth hypersensitiveness. Lower concentrations are more recommendable.

When the desired result is accomplished /this is clear several days after the end of procedures – teeth are with natural color/, the teeth are treated with fluor, and if it is necessary the fillings of the front teeth will be exchanged /with brighter ones – photopolymer is not affected by the gel and remains more yellowish/.
The effect lasts 3 to 5 years, but the teeth never completely resume their color.

There are teeth however, which are resistant and are slightly influenced by the procedures. There is an indication for the next lower invasive step – considering and making of porcelain facet.
